Abstract
As a member of the NASA Aerospace Safety Advisory Panel, Ms. Grubbe is one of 8 people chartered to give advice to the NASA Administrator and to the United States Congress with regard to safety in human space flight. Her remarks will focus on significant incidents within NASA and draw parallels and lessons learnt to events within the chemical, bio and petroleum industries. Instances from the Apollo and Space Shuttle programs will be highlighted.
