DESCRIPTION:As the patient safety agenda continues to shine the spotlight 
 on the number of errors in clinical medicine\, most radiotherapy departmen
 ts can point to excellent quality assurance and incident reporting systems
  as evidence that we are among the safest disciplines. QA is especially im
 portant in the face of increasing physics accuracy and computational power
  which together mean we can deliver known doses of radiation to our target
  volumes and away from critical structures with millimetre accuracy. With 
 increasingly precise radiation delivery it becomes even more important tha
 t we define our targets accurately.
