 ttish prison custody identified failure properly to secure evidence\; proc
 urator-failure to allow the full extent of post-mortem\; sherrif-failure p
 roperly to report actually-prescribed medication-dosages\; failure to lead
  admissible empirical evidence\, for example in reports by HM Chief Inspec
 tor of Prisons for Scotland\; reliance instead on subjectivity\; long dela
 ys in providing written determinations\;&nbsp\; inconsistency in the deter
 minations reached on similar cases\; failure to post all written determina
 tions on Scottish courts&#39\; web-site\; failure to consider the epidemio
 logy of prisoner-deaths in deciding that a written determination should be
  made. <br> <br> For Scotland\, most of these failures have been taken int
 o account\, and remedied\, by recommendations in the report of the Lord Cu
 llen.<br> </span>
